Members can designate a beneficiary at any time while they are an active, deferred or retired member. Members must complete two beneficiary forms: one for the defined benefit component and one for the defined contribution component of their plan.
Log into your account to complete the Beneficiary Designation Form for the defined contribution component.
Complete the Designation of Beneficiary Form (VRS-2) for the defined benefit component.
